• PRN: 10 hours weekly to start; mostly resource navigation for now.
• 1041 Old US 52 B, New Richmond, OH 45157 (Next to Rivertown IGA)

• $25–$40/ hour (negotiable)

EXPECTATION / REQUIREMENTS

Assessment & Planning

• Conduct comprehensive psychosocial assessments of participants and caregivers to identify needs, strengths, challenges, and goals.
• Develop, implement, and regularly update individualized care plans or counseling plans based on assessments and person-centered goals.

Counseling & Support

• Provide individual, family, or group counseling to address social, emotional, and adjustment needs.
• Facilitate support groups for participants and caregivers (if applicable).

Care Coordination

• Collaborate with interdisciplinary team members (nursing, program staff, therapists, case managers) to support participants holistically.
• Coordinate with community resources and refer participants/families to external services (housing, legal assistance, transportation, support agencies).
• Ensure social goals and psychosocial needs are integrated into the participant’s service plan.
