EXPECTATION / REQUIREMENTS

• Complete start of care assessments only
• Primary focus on patient care
• Accurate, timely documentation
• Conflict resolution skills
• Critical thinking skills
• Motivated self-starter with an approachable, friendly, and positive attitude
• OASIS Assessments
• PDGM – Must have worked in this newest payment model
• Professional license – Current and unrestricted
• Strong communication, organizational, and time management skills
• Willingness to learn and help support other team members
• Monthly virtual team meetings
• Ability to attend 2-3 mandatory in-service/skills labs in person per year
• Reimbursement is provided for time and travel
